Actor Release Form
Actor Release Form is a document that shows the actor allows the institution/organization to complete permission and rights to the captured videos or photos. This document is normally given to all talents before the production starts to avoid any issues in the future.
This Actor Release Form template contains the terms and releases agreement that the actor needs to agree to. It also has small information about the actor and a widget that capture digital signatures that are called the E-Signature widget. This widget will capture the actor's e-signature and the producer's e-signature showing that both parties agree to the release agreement. This form template is also using the Narrative widget that allows you to have blank fields together with static paragraph texts. This template also has a Print Form function just in case you want a hard copy of the release form.
