Now, coming back to your question, I've cleared your form caches; your forms are now refreshed. Can you update the widget first, then upload an Excel file? Let me show you how to update the widget:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on the Spreadsheet to Widget.
2. Choose Wand Icon to open the Widget Settings.
3. On the right side, scroll down and click on the UPDATE button. That's it.
