Thanks for reaching out to Jotform Support. I understand you would like a QR code in every submission PDF. Just note that once you added a QR code to your form it will automatically be attached to your PDF file. You will have to make a test submission to check out the result. Our Dynamic QR code widget lets you generate unique QR codes on your form based on each form user’s previous answers. Check out the example in the screencast below:

Thanks for getting back to us. I’m sorry you're having difficulties with this. Could you please tell us why it wasn't working on your PDF? Can you also please clarify what you want to achieve so that we can look for a suitable workaround for you? Please keep in mind that the Dynamic QR code widget allows you to generate unique QR codes on your form based on each form user's previous answers. When you scan the QR code, it will reveal the details that you encoded to the widget, such as the email address entered by the form submitter. In addition, the QR code will be automatically added to your attached PDF in each submission.
