Form docuemnt is not changing

    I followed the instructions above, but my Word Document (after I submit a form) looks the same as the original (with the curly brackets). I can't figure out what I am doing wrong - any help is much appreciated.

    Even i m facing the same problem. The value is same with bracket. It is not getting replaced with actual form data
