Conditional Fields: Adding Drop Down or Single Choice Fields Alongside Name, Email, and Phone

In addition to name, email, and phone, can I add either a drop down or single choice field to these conditional fields?

Thanks for reaching out to Jotform Support. I can see this is related to this support ticket that you opened. Going back to your question, the answer is yes. You can also use the Dropdown element, asking for the number of guests. Then, depending on the number of guests the form elements will show. Let me provide a demo to you:

Add a Dropdown element, options, and assign values:

1. In Form Builder, click on the Add Element button on the left side and drag and drop the Dropdown element in whichever part of the form page.
2. Now, click on the Gear icon on the right side.
3. Next, in the Dropdown Properties, add the number of guests inside the box below the Options.
4. Then, enable the Calculation Values and assign an amount for each option.

Add a Form Calculation widget to get the value for the selected number of guests:

1. Click on the Add Element button, and go to the Widgets tab.
2. Drag and drop a Form Calculation widget on the bottom part of the form.
3. In the Form Calculation Widget settings on the right side, click on the Add Field button
4. Select the Dropdown element and click on the Save button.

We'll now create Show/Hide Field conditions to show the form elements depending on the number of guests:

1. In the orange navigation bar on top, go to the Settings tab and select Conditions from the left menu.
2. Now, create Show/Hide Field conditions.
3. Finally, click on the Save button for each.
