Form Submissions Download: Missing Data in Excel Export After March 20th

    Hello

    I created a form and I download the respected signups weekly. We end it around march 20th, now when I try to download the submissions now it only populates this:

    Before it would populate Student name, grade, serial number, parents name, their phone and email. I do not think I messed with any settings or certainly did not mean to.

    Is there someone that can help get this changed back. The bottom image shows the last download I did and what it populated via excel.
