The title of the form isnt changing on the title tag

I cloned one form and changed the title for the form.
When I open the link, the title shows as Clone of 6th HIS - Morocco - Sponsor Attendee Details - 5 when I have changed the name to 6th HIS - Morocco - Sponsor Attendee Details - 6
