    Hi Oleg,

It looks like you have shared a link to a Jotform app with an embedded form or document related to an electrical proposal. However, I am unable to access or view the content directly from the link you provided.

If you need assistance with this form or document, could you please provide more details about the issue or question you have? For example, are you experiencing problems with the form functionality, submission, or something else?

Once I have more information, I'll be glad to assist you further.

That definitely sounds like a phishing attempt, and you did the right thing by stopping there.

Please report this app/form to us so our team can review and take action:

    • Go to the Jotform Abuse Report page: https://www.jotform.com/report-abuse/
    • Scroll down to the report form.
    • Fill in the following details:
      • Your full name
      • Your email address
      • The link of the form/app you are reporting (the URL you shared)
      • Your reason for reporting (e.g., “Download button redirects to a phishing site asking for O365 password”).
    • Submit the form.

You can also review the full guide here: https://www.jotform.com/help/what-to-do-when-you-feel-a-jotform-form-is-used-in-a-scam

If you can, avoid entering any credentials and consider changing your O365 password if you interacted with that page at all. Thanks a lot for helping keep Jotform safe for everyone!
