    my other question is do you furnish the template that I can insert into our church website?

    Hi Reed,

    Thanks for reaching out to Jotform Support. If you're referring to embedding your form to your website, you can embed your Jotform form directly into your website. You can choose Embed and then copy the code for your site. The available options include JavaScript, Source Code, and iFrame. Let me show you how:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
    2. In the menu on the left side of the page, click on Embed.
    3. Now, select the embed option you want to use, and click on Copy Code. (I used the iFrame method only to show in this case.)
    4. Paste your code into your webpage's embed area, and that’s it.

Image-1 If you are using a platform like WordPress, Wix, or Squarespace, look for a block or widget that accepts custom HTML or code embeds, then paste the code there.
