Singed forms and quizzes

    Hello Jotform Team,


    I run a youth sports business with ~50 part-time coaches and I’m looking for a simple system for onboarding and annual compliance.


    I need to:


    • Collect 3–5 new e-signatures per month, plus a yearly re-sign event (~50 coaches in May)
    • Host/attach policy documents and training videos for staff to review
    • Include short auto-graded quizzes for compliance training
    • Create a simple annual certification flow (review → sign → quiz → submit)
    • Track completion status and export or integrate with Monday.com



    Staff are part-time and need a very simple mobile-friendly experience.


    Can you recommend the best plan for this use case and any limitations I should be aware of (especially around signature volume and quizzes)?

    Hi Justin,

    Some limits renew on a monthly basis, like form submissions, signed documents, etc. Also, some limits apply to your whole account, like total submissions, upload space, number of forms, etc. So if you need more than 50 forms, then I will suggest the Gold plan. But if you need less than 25, then I will suggest the Bronze plan. If you buy the Bronze plan and you reach any limit, you can upgrade to Silver or Gold plans at any time.

    Monday.com integration is available in all plans, so I would suggest starting with the Bronze plan yearly plan. You can also check our account limits and usage guide for more clarity and a better understanding.
