    Hello I’m unsure what to upload for document verification. I uploaded my drivers license and a photo of myself but I got declined. I have 3 forms in one of them said I need verification to be able to use or it’s gonna be disabled. I don’t want it to be disabled so I’m trying to figure out what’s needed. I don’t own a business or anything to be able to show documentation. These three forms I created are for personal use.

    Hi Jesse Gallup,

    We reviewed your KYC submission, but we couldn't verify your account as we need to verify your affiliation with the organization as well. Can you check the Data section of your Account Settings and fill out the Additional Document Form? If the form says, you can only submit it once, you can upload anything requested as images here on this thread or send an email to us at kyc-review@jotform.com with the subject Ticket #37449081 and include any requested information.

    We'll be able to help you once you submit the form.
