    Hi Heidy,

    To pull up the right account, can you share any form URL created under the account? If that's a paid account, you can also share the following details with us so we can locate the account where the charge occurred.

    • The last 4 digits of the card
    • The expiry date of the card
    • Transaction date
    • Amount charged
    • Card type (whether it's Visa, MC, or Amex)
