Form Building: How to Add Item Options and Note Box to Catering Order Form

Hello, I am needing some assistance with adding items to my catering ordering form. I have 6 different "party packs" listed on my form but I want to add options underneath each package listed to offer 1 lb of pork or 1 lb of mac and cheese etc. added on to their pack if they think they need more. how can i do this? i also need to add an empty note box for them to add details that we havent listed. how do i do this?

Thank you! I got this one set up how I want, however, I want to be able to add pricing to the add ons. I.E. Where it says +1 lb pulled pork i need to add 6.00 to that area and so on for each add on. how do i add pricing for each of those options

I have a pack on my form that is the same exact thing but you can either pick pulled pork or pulled chicken for the meat. is there a way i can combine these two products into the same product on the menu but have a box for them to select pulled pork or pulled chicken for the meat? they are the same price
