Form Building: Creating a questionaire form

    I would like to speak with someone concerning building a questionnaire for my business website. The questionnaire for Wills, Living Wills, Trust and Pour Over Wills which would produce a detailed document(s).

    If possible I would like to speak to someone in person by telephone.

    My website is almost complete so I'm looking to get this done soon.

    Hi Steven,

    Thanks for reaching out to Jotform Support. We don’t offer phone support, but I’ll guide you through everything right here. About creating a form specifically questionnaires, Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. Dragging and dropping features or elements makes it really easy to create forms. I'll help you do it:

    1. On your Workspace page, in the top-left corner of the page, click on Create.

    2. Then, click on Form and, select Create From Scratch.

    3. On the next page, select Classic form if you want to show multiple form elements on a page, and choose Card form if you want the form elements shown individually on each page of the form.

    4. In Form Builder, click on Add Element in the left corner of the page to see the complete list of Form Elements, Payment Gateways, and Widgets.

    5. In the menu that opens, drag and drop the Form Elements, Payment Gateway and Widgets you want to use.

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
    2. In the menu on the left side of the page, click on Embed.
    3. Now, select the embed option you want to use, and click on Copy Code. (I used the iFrame method only to show in this case.)
    4. Paste your code into your webpage's embed area, and that’s it.
