You can absolutely generate a new QR code that points to last year’s form.

If you want a QR code that simply links to that existing form:

    • Open your form in Form Builder.
    • Click Publish in the orange bar at the top.
    • Under the Share Form section, click QR Code.
    • Click the Download button to save your new QR code image.

You can also see more ways to share forms and assets via QR in this guide: Generate QR Code to Share Your Forms/Assets.

If instead you want to create a QR code inside the form (for people to scan from the form itself):

    • In Form Builder, click the Add Element menu on the left.
    • Go to the Widgets tab.
    • Search for QR Code and drag it into your form.
    • Configure it as needed and save your form.
    • Full setup steps are here: Add Scannable QR Code on Your Form.

If you want users to scan a QR code from within your form (for example, to capture a code from a badge or ticket):

    • In Form Builder, open the Add Element panel.
    • Go to Widgets.
    • Search for QR Code Reader and drag it into your form.
    • Adjust the widget settings and save.
    • Detailed steps are here: Add QR Code Reader Widget to Your Form.

And if you need a QR code for a website, social media page, or file (like a PDF) instead of a Jotform form, you can use our standalone tool:

If you tell me whether you’re reusing the exact same form link from last year or creating a brand-new form for this year, I can tailor the steps more specifically for you.
