- Open the form in Form Builder.
- Click the Paint Roller icon on the right side to open Form Designer.
- Go to the Themes tab.
- Select any theme and click Use Theme.
This process resets the form's styling and can remove stuck or missing images issues.

Are you referring to this form? I've cloned this form and made a test submission. Then, I went to the PDF Editor to view the PDF file for the submission. There are multiple PDF files created. Can you tell us which of those is the same as the PDF file shared on your screenshot? Here's a screenshot of my cloned form PDF Editor:

We tried cloning your form and tested filling out the cloned version. The image from the mentioned PDF was retained correctly after I downloaded it. I also downloaded the most recent submission from your PDF Editor, and it seems to be working fine as well.
