You can achieve it by using a Multiple Text Fields widget. Let me show you how:
1. In Form Builder, click on Add Element on the left side of the page.
2. In the menu on the left, click on the Widgets tab.
3. Under the Widgets tab, drag and drop Multiple Text Fields widget to your form.
4. Now, in the menu on the right, under the Fields section of the General tab, remove the default sub-field names.
5. Next, enter the desired sub-field names in separate lines.
6. Then, name the Multiple Text Fields widget as desired.
