Show Multiple Form Fields Based on Answer

in my form "Downtown Housing Development Grant Program Application" I want 4 new questions to SHOW, IF yes is selected. I have created the condition, but I can not select all 4 questions they are questions 7.8.9. and 10. if will only let me select 1, i tried shift.down arrow, but that does not work and i deas?
