Zapier Integration: Allowing multiple forms to be selected for Zap trigger

I'm just after some help, I have made all of my consultation forms on jot form for different beauty treatments, I can work out how to connect them to my set more booking app using zapier as the trigger app but I can only see that it lets me connect one form and the trigger being a new appointment made. however I feel like I need to filter this more as I offer multiple services so I would be needing to send a specifict consultation form for the treatment being booked, is this something I can do? thank you x
