It appears your form is disabled. You can enable your form in a few simple steps. Let me show you how: 

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings
2. Then, in Form Settings, under the Form Status section, click on Disabled.
3. Next, in the window that opens, click on Disabled and select Enable. 
4. Now, click on Save. That’s it. You’re done.
