Form Submission: Church Street form opens as Holy Family's form

I created a form for Church Street, but when I test it by sending to myself, it opens up as the Holy Family's form which is different. Not quite sure what is happening.

But when I send it as a reminder email, it says

"A form is waiting for you to fill out.

but when I click onto the "View Form", it opens up the HFI Weekly Order form as seen below

Can you try to remove the Reminder email and recreate it? It's really easy to delete. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
2. Click on Emails on the left side of the screen, and select the Schedule A Reminder Email option.
3. Hover your mouse on the Reminder email and click the Trash icon.
