Form Builder: Page Image not updated

Two issues:

1. In Form Designer, the Page image shows correct but on the Built screen it is something different. When the link is pulled up, the correct picture is shown.
2. The form shows in center of page on the Build screen but when you pull up the link it is over to the right of the screen.
