I'm trying to create a mock form so I can propose to my company.

I'm looking to create an order template that our sales reps can provide to customers. The form needs to have the capacity of as many as 50 product lines. It needs to have at least 5-10 dropdowni or columns of choices for them to check off for their customization. Also need a fill in option if they want something specific. I'd like to incorporate images so we can use this in the 5-10 dropdown to help match their specfic request

I don't see any templates that resemble this. I'd like them to see their answers so I don't want it to be a closed question. Some questions can be fill in or check box.

    For Example:

    Metals Markers

    A. Duo

    Metal Type

    Color Fill Background

    Color Fill Text

    Text Front

    Text Back

    Image

    Other Information

    B.

    C.

    Hi Myrna,

Thanks for reaching out to Jotform Support. I'm an AI assistant here to assist you.

I have created a custom product order form template based on your requirements. It includes capacity for up to 50 product lines, multiple dropdowns for customization, fill-in options, image incorporation in dropdowns, and a mix of fill-in and checkbox questions so your sales reps can provide it to customers and customers can see their answers.

You can access and customize the form here: Custom Product Order Form

Give it a try and let us know how it goes.

    Hi Myrna,

This is the Custom Product Order Form created by the AI Agent, Podo. May I know which properties you are referring to? If you're referring to the Product Lines field, it is currently at the bottom of the form, below the Submit button. You can simply drag and drop it to move it to the correct location.

I’ve gone ahead and moved your other question to a separate thread to keep things tidy, and someone from our Support Team will help you out with it here.
