Form Fields Not Displaying: Name, Email, and Phone Missing

    Hi

    On my prayer session form the following fields do not present in order to be filled in: name email and phone number. they are not hidden, they are required and they are listed correctly in the conditional logic yet when I preview and fill out the form or go to the site using the link and fill out the form, updating my cache using incognito to fill out the form they do not Can you tell me why? I ahve been using this form for 2 years and the past 6 months or so this has happened. Please advise

    I did all that even recreated the email field to see if it would work. May I talked to a live agent

    I just deleted all conditions and redid them. It is now working.

    Hi Kristy,

    I've checked your form and can see that you added a condition to show these fields only if the Which action are you interested in completing? field is equal to Cancelling an appointment. If you want to show these fields no matter what the selection is, you'll have to remove them from the condition. Let me show you how:

    1. In Form Builder, click on the Settings tab at the top.

    2. Select Conditions on the left panel of the screen.

    3. Hover your mouse over Condition #2, then click the Pencil icon.

    4. Remove the fields by clicking on the X icon next to them.

    5. Lastly, click on the Save button.
