I want to create a new from that contains data from the first three forms on the attached website, I want to know if it's possible to map it to the form that was downloaded as a pdf and then we can use this as a response elsewhere

Podo: Support SpecialistHi Ayomide,
Thanks for reaching out to Jotform Support. I'm an AI assistant here to assist you.
You can create a new form that contains data from multiple forms, but the process involves some steps and considerations:
Combining Data from Multiple Forms:
Jotform allows you to combine submissions from different forms into one file using the Combine Submissions app. This app merges data after collection, but combined submissions will appear on separate tabs. Additionally, you can connect multiple forms in Jotform Tables to manage submission data from several forms in one place, allowing for lookups or rollups with real-time updates.
Mapping to a PDF Form:
Jotform Smart PDF Forms lets you import your PDF document and convert it into an online form. You can upload your PDF, map the form fields, and collect responses online. The online form fields can be linked to the PDF fields, allowing you to download filled-out PDFs or use the data elsewhere.
Here’s how to do it:
- Go to your My Workspace page and click on Create.
- Select Form and then choose Document to Form.
- Upload your PDF file.
- Adjust field connections manually if some fields are not detected automatically.
You can then map the fields on your online form to the PDF document fields. This mapping allows the data entered in the online form to be reflected in the PDF version, which you can download or use as a response elsewhere.
For detailed guidance, you can check out the following resources:
- How to Link Your Online Form Fields with PDF Through Smart PDF Forms

Keep in mind that some field types cannot be mapped to PDFs, such as Appointment, Product List, Image, File Upload, Captcha, and some widgets.
Let me know if you need help with any specific step or feature.
Give it a try and let us know how it goes.
