    Hi Anne,

Thanks for reaching out to Jotform Support. I'm an AI assistant here to assist you.

Saving your form as a template only takes a minute to set up, here's how:

    1. In Form Builder, in the orange navigation bar at the top, click on Publish.

    2. Next, in the Share Form section, click on View More.

    3. Click on Share as a Template.

    4. Enter a Template Title, a Description and choose Language.

    5. Then, select a Category for the form, and click on Share.

Once you're ready to share your form, you can copy its URL from the address bar. Check out this screenshot below:

Keep in mind this is designed for you to save templates for your own account, and only you can access them. You can reach your template page using this link: https://www.jotform.com/form-templates/user/{yourUsername} replacing the last part with your username. If you want others to use your form template, they’ll need to clone your forms instead. We have a guide here about How to Clone an Existing Form from a URL to learn more.

Reach out again if you have any other questions.

    Hi Anne,

Are you trying to integrate the form into your Microsoft Word? If yes, take note that we don't have a direct integration to Microsoft Word. As a workaround, you can use a third-party integration like Zapier to integrate the form into your Microsoft Word.

Give it a try and reach out again if you have any other questions.
