    Hi Jessica,

    Thanks for reaching out to Jotform Support. I checked your form and see that there are no fields in it but the paragraph fields. I suggest collecting information required for your case, such as the Name field. Let me show you how to add that:

    1. In Form Builder, click on the Add Element button on the left.
    2. Next, click on one of the elements like the Full Name field and drag it to your form.
