Thank you for bringing this to our attention. Do you have a Jotform account with us? Do you have forms that are open for submission?

If you don't have an account with us, I can send it up to our higher level agents and get your email completely deactivated from Jotform. It may take a few days, but it will ensure that there is no account tied to your email and therefore will not be able to send out messages to your contacts.
