How can I create a QR code for registrations and have the form send it to ticket holders?

I have created a form called Canadian Plains youth which was supposed to generate a QR code for the number of registrations, but no one is receiving the QR codes. I need help creating a QR code and configuring the submission form to send the QR code to the ticket holders. I have not mentioned any specific steps I have tried yet. I am requesting human assistance to create this setup.

I have one on the form, but it isn't showing, and people have not received one in their email either. pls advise

Thanks for using Jotform. I checked your form and I can see that the Dynamic QR Code widget is added, but the label CPDY26 Ticket you mentioned is not present in the form. Could you please provide more information about what exactly you would like to display in the QR code?
