wroccreativeLet's say I'm putting together an order form, where customers can select as many items from the list as they'd like to order. But, for each order they select, I want a "comments" field to appear on the right for any customization requests. I don't want the comments field to show up unless that menu item is selected.
On submission, the answer to this question would ideally be formatted as
Item 5: Extra cheese
Item 12: Medium rare
Item 14: No cherry
What's the closest approach to what I'm seeking? -

Thanks for reaching out to Jotform Support. The workaround I will recommend, is to use Short Text fields that will only show once they select the option on the Multiple Choice field. Let me show you how:
1. In Form Builder, click on the Add Element button and add multiple Short Text fields to the form.
2. Then label the fields accordingly.
3. Now go to Settings tab on the top, then go to Conditions tab on the left side panel.
4. Select Show/Hide Field condition from the options.
5. Set similar condition for each option like the one below:
