In addition we require a Certificate JOTFORM, which you can obtain from the UK Revenue Agency.
Both of the documents we request are necessary in Portugal, because the service your company provided it is subject to withholding tax in Portugal.
In order for us not to pay that amount here, we need your signature on the document attached below and your residence certificate.
When you open the document, on page 2, you will see the box with "VIII", please sign there and return the document scanned as well as the residence certificate of the company.
