If you have a QR code from Jotform that is no longer working, it's important to know that QR codes generated for forms do not expire. However, if the QR code is linked to a form that has been deleted or the form URL has changed, the QR code will no longer work.
Unfortunately, you cannot reuse the exact same QR code to attach it to a different form. Each QR code is uniquely generated for a specific form URL.
Here’s how you can create a new QR code for your form and use it on your business card:
- Open your form in Form Builder.
- Click on Publish in the top orange navigation bar.
- Under the Share Form section, click on QR Code.
- Download the new QR Code and replace the old one on your business card.
If you want to add a QR code directly inside a form, you can add a QR Code widget from the Add Element menu under Widgets.

To check if your form is still active and not deleted, you can do the following:
- Go to your Workspace page on Jotform.
- Look for the form in your list of forms. If it is not visible, check the Trash folder located at the lower left side of the Workspace page.
- If the form is in the Trash folder, you can restore it by selecting the form and clicking the Restore option.
If the form is not deleted but the QR code is not working, also check if the form is enabled or disabled. You can enable or disable the form from the Workspace page or within Form Builder settings.
Here’s how to enable or disable a form:
- On your Workspace page, hover over the form and click More, then select Enable or Disable.
- Or in Form Builder, go to Settings, then Form Settings, and adjust the Form Status.
Make sure the form is enabled so the QR code link works properly.
