How to Remove Agent Error From Screen?

    I'm trying to edit my Scholarship Application Form under a tight deadline. Unfortunately, this "Agent Failed" error image appears on the screen, blocking a portion of the form and making it difficult to edit and/or read the form. Please help me remove it, so that individuals attempting to complete my form do not see it. Thank you so much for your prompt assistance.
