My Template Diappeared

Hello- I created an apparel order template and need to edit it, but it has disappeared from my template lists. Although, the link I sent out still works. https://form.jotform.com/260764432133149

I checked your form, and it's still active but on a different account. The form is under m*l*s*a.*m.*e*c*@gmail.com. If you're familiar with the email, you can log into that account to see the form.
