Smart PDF Tool: Last Pages Not Included in Online Form

We uploaded a PDF using the Smart PDF tool and the last several missings are not included in the form. Please advise as to the solution to include the remaining pages.

I've cloned your form and confirmed there are only six pages. Did the Smart PDF Form show an error when you imported the PDF file? How many form pages do you expect the Smart PDF Form to show?
