- Hover over your Smart PDF Form and click Edit Form.
- On the Smart PDF Forms page, click Upload in the blue navigation bar.
- Click Update PDF and then Re-Upload PDF to upload the new PDF with all pages.
3. PDF Connection Settings: Ensure that your Smart PDF Form is connected properly to the original PDF. You can check the connection in the Form Settings under the Original PDF Connection section.
4. Field Import: Note that Smart PDF Forms import fields from the PDF to create the online form but do not copy the design. Make sure the fields on the missing pages are properly recognized in the PDF.

I've cloned your form and confirmed there are only six pages. Did the Smart PDF Form show an error when you imported the PDF file? How many form pages do you expect the Smart PDF Form to show?
