Thanks for reaching out to Jotform Support. Can you check the screenshot below if that's how you want the confirmation email to appear?

If yes, let me show you how:
First, let's add a short text element to show the $35 fee.
1. In Form Builder, click on Add Element on the left side of the page.
2. Then, in the Form Elements menu, under the Basic tab, scroll down the list and choose the Short text field.
3. Now, just drag and drop it to your form.
4. Then click on the Gear icon to access properties.
5. Under the Short text properties, enter the Field label.
6. Click on Advanced, in the Default value enter $35 and toggle on Hide field.
Second, let's add a condition.
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Next, click on Conditions on the left of the page.
3. Now, click on Add Condition and then click on Show/Hide Field.
4. Set the conditions as you see below and click on Save:

2. Now, in the menu on the left, click on Emails.
3. Then, hover your mouse over the Autoresponder Email, and click on the Pencil icon.
4. Go to the Email Content and add an Extra row to add the field for the extra fee to Spay/Neuter.
5. Enter the Field name and click on the Form fields at the upper right part of the email.
6. In the menu, select the Extra fee to Spay/Neuter and click on Save at the lower part of the email.
