one of my forms was disabled because it was flagged for collecting login information. I believe this was a mistake because we are not collecting login info, the field was supposed to be a code for users to enter specifically to access certain parts of the form

- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.

Though the field we were using is now deleted in order to reactivate the form. It was a short text field that asked users to enter a password, which was only to restrict access to certain areas of the form to certain staff members. It was not a password to access any account, just to allow specific actions on the form only to be taken by management, as this form serves many purposes across multiple departments.
This is now our 3rd time a form has been disabled for a similar reason (not this specific form). I understand based on terms of service that continued violations could result in permanent account deletion. Our past violations also seemed to be a misunderstanding, though we now know what not to include in our forms to avoid being flagged in the future. If in the future our account was permanently disabled and the violation was found to be a mistake, would it be possible to recover our account by contacting support?

I checked the form and it's working fine now that you've removed that field. I understand that you were using the field for internal staff access and not to collect login info for external accounts. Our system automatically flags words like password or login to help prevent phishing.

To avoid this happening again, it's best to use labels like Access Code or Management Key instead. You'll find more info about our phishing policies in Jotform's Terms of Use.
