Hello,
An on boarding form was created and QR code generated so that anyone could scan it and fill it out. We have one individual who claims to have scanned the QR code and was taken to a pre filled form by another prospective employee.
I need to understand either how this happened or if it is even possible for it to happen.

To avoid this, ensure that the QR code points to the standard blank form URL without any prefill parameters. You can generate a QR code directly from the form's Publish section under the Share Form area, which will link to a blank form.

This has already happened. The form that was created was not pre filled.
One person scanned the QR code and filled in their information, and another person scanned the same QR code and allegedly was met with a form pre filled with another applicants information.

Based on Jotform's functionality, if the form was not set to pre-fill, it should not show data from a previous submission when accessed via the same QR code. Each scan of the QR code should lead to a blank form for new entries.
However, there are a few scenarios that might cause the behavior you described:
- If the QR code URL was modified or included prefill parameters by mistake, it could show prefilled data.
- If the form was accessed on a shared device or browser where the previous user's data was cached or saved in the browser's autofill, it might appear prefilled.
- There might be a misunderstanding where the form was actually shared with a prefilled URL rather than the blank form URL.
Jotform forms do not automatically save and show previous submission data to other users unless prefill settings or URLs are used.

1. On the Help Center page, scroll down to the Your Answer section and click on the Image icon.
2. Drag and drop your image into the Upload box, or click on it and select your file.
3. Then, click on the Add button in the bottom-right corner of the window.
4. To resize it, click on your screenshot and then click on one of the boxes in the corners, and drag it inward.
5. Once you're finished, click on the Post Answer button at the bottom right of the Your Answer section.
