Reymae JotForm SupportHi Nancy,
Thanks for reaching out to Jotform Support. In order for us to assist you further, we’ll need a few additional details related to the transaction. Can you send us the details below to help us locate the account where this charge was incurred:
- The last payment amount:
- The last transaction date:
- The last four digits of your card:
- The expiration date of your card:
- The type of card (Visa, Mastercard, Amex, etc.):
Once we hear back from you, we'll be able to help you with this.

southfieldmentalhealthThe payment was $348, but it shows up four times. Two of them look legitimate and two don't look legitimate so our bank flagged them.
- 348
- 2/20/26
- 1729
- 11/29
- Mastercard
The debit still shows in our bank, but we cannot log into Jotforms. Why not?

Reymae JotForm SupportHi Nancy,
I've gone ahead and reached out to our relevant team to check on your account. We’ll get back to you as soon as we get an update from them.

Reymae JotForm SupportHi Nancy,
I checked with our team regarding the payment attempts on your account. Based on the review, all four attempts are indeed related to Jotform.
However, as seen in the screenshot you shared, it appears that more than one payment processor may have been involved. This can sometimes result in different descriptions appearing on your bank statement, which may explain why some charges look unfamiliar.
If your bank flagged these transactions, you can let them know that the attempts are associated with your Jotform payment, but feel free to reach out if you need further clarification from our end.
Reach out again if there’s anything else we can help you with.
