Add a cover page to my form

Hi,

I made my form by using a Google Forms template, but I can't figure out how to add a cover letter to it. I know that I can't add a PDF letter, and that's not what I'm looking for. I'm looking for how to add a welcome card, but in the FAQ I could find it showed a box that I don't have. Any help is appreciated!

Hi there - Unfortunately, it isn't possible to add a cover letter in a regular classic form. As a workaround though, you can add a page break before your first page and add your cover letter to it. Example:
