This does not resolve the issue I am having. Despite following these instructions, my form preview continues ot display the incorrect name. I've cleared my cache in JotForm, logged out several times, and tried publishing the form separately as well. Nothing works.

Thanks for reaching out to Jotform Support. I understand that you need to update the Page Title of the Form. Can you send us the URL link to the Form?

Lets' walk through the steps on how to locate the URL link to the Form together.

1. On the Form Builder page, click on the Publish tab on the orange navigation bar at the top. Then, click the Copy Link button.
   

In the meantime, can you try the steps below to update the Form title? Let's walk through the steps on how to do that.

1. On the Form Builder page, click on the Settings tab on the orange navigation bar at the top. Then, update the Form Title.
   
2. Next, click the Show More Options button. Then, update the Page Title.
