Thanks for reaching out to Jotform Support. Generally, the IP address gets blocked when an account is suspended by our anti-phishing system. This isn't necessarily your account. There may be another user who may have created a Jotform account using the same IP address and created a form violating our terms of usage. Can you check the IP address using this website? Then, respond with the IP address.
Once we hear back from you, we'll be able to help you with this.
