How to create a Quiz form with playbook recommendations?

I’d like to set up a quiz funnel using JotForm and need some guidance. The main heading for my quiz will be:

“Not sure where to start? Take the quiz and get a personalized path with 3 playbooks to match your goals.”

Here’s what I’m trying to achieve:

1. A user takes the quiz.
2. Based on their answers, they are matched with 3 recommended playbooks that fit their goals.
3. The results should be shown at the end of the quiz and sent to their email.

Could you please explain how I can set this up in JotForm? Do I need conditional logic, product recommendation features, or integrations to make this work?

Thanks for reaching out to Jotform Support. You can either manually create your own quiz form using the form elements or you can use Jotform's Quiz AI Generator. If you wish to do it manually, Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. Dragging and dropping features or elements makes it really easy to create forms. I'll help you do it:

1. On your My Workspace page, in the top-left corner of the page, click on Create.

2. Then, click on Form and, select Create From Scratch.

3. On the next page, select Classic form if you want to show multiple form elements on a page, and choose Card form if you want the form elements shown individually on each page of the form.

4. In Form Builder, click on Add Element in the left corner of the page to see the complete list of Form Elements, Payment Gateways, and Widgets.

5. In the menu that opens, drag and drop the Form Elements, Payment Gateway and Widgets you want to use.

Now, to show the specific results at the end of the quiz, you can use Conditional logic to do that. You can use the Change Thank You Page condition since it allows you to customize the Thank You page message based on how respondents interact with your form. 

Lastly, create multiple Autoresponder Emails for each result, and conditionally send the specific Autoresponder Email that matched the 3 recommended playbooks that fit their goals. You can use the Change E-mail Recipient condition to do that. Let me show you how to create an Autoresponder Email:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. In the menu on the left side of the page, click on Emails.
3. Now, click on Add Email.
4. Then, select Autoresponder Email, and customize the options the way you want them, and you’re done.
