    Unfortunately the widget does not open. It only shows a very small portion of an image of the form

    Hi there - I believe you're referring to the iFrame widget on this form. https://www.jotform.com/81564729949980.

    Per checking, the URL that supposed to show in the iFrame is deleted already.

    Kindly try updating the Frame URL to a valid one.
