Google Drive Integration: Creating a file name with two numbers

I need to create form for collecting photos on Google Drive, but the form will collect two numbers: 1) facility code, 2) equipment code. Is it possible to create file name with two numbers: facility code_equipment code?

Now I have indicate such rule, but there is incorrect result: facility code_facility code.

How to solve it?

I couldn't find an account linked to the email address you used on this thread, can you share us the email address on your Jotform account or the link of the form you're working so we can assist you further? You can easily find your form's URL in Form Builder. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
2. In the Quick Share tab, click on Copy Link in the Share With Link section.
