Hi JotForm Team,
I am trying to upload our Excel file for doing the auto populate using the Spreadsheet to Form Widget but when I upload the file, it sends me this message "Screenshot attached".
Could you please help me on how to work on this?

Regarding the Spreadsheet to Form Widget error you encountered when uploading your Excel file, please ensure the following:
- The widget only accepts XLS or XLSX file formats. Other formats like CSV or Google Sheets are not supported.
- Make sure the column names in your spreadsheet exactly match the field labels on your form for the data to populate correctly.
- The unique code you use for searching is case-sensitive and must match exactly.
Here’s how to set up the widget properly:
- In Form Builder, click Add Element and select the Spreadsheet to Form Widget under Widgets tab.
- Upload your Excel file (XLS or XLSX) in the Widget Settings.
- Select the Access Code column from your spreadsheet to use as the unique code.
- Configure the widget settings like button text and messages.
- Click Update to save the widget configuration.
Make sure your Excel file meets these requirements and try uploading again.

I did all the troubleshooting as well as clearing my cache but its still now working.
Can you try using this form?

Hi Jerico,
We are not allowed to edit a user’s form without permission, and we also do not have access to your Excel file. You can try adding a new Excel file to the widget instead. Make sure to set the settings correctly by choosing the Access Code Column properly. Let me show you how to upload a new Excel file to the Spreadsheet to Form widget:
- In Form Builder, click on the Wand icon next to the Spreadsheet to Form widget to open Widget Settings.
- In the Widget Settings panel, click on the General tab.
- Click on Upload File and select your new Excel file from your computer.
4. Once uploaded, make sure the Access Code Column is set correctly before clicking Update.
