Thanks for reaching out to Jotform Support. Let me help you with that:

To remove the form you mentioned, you can delete it by following these steps:

1. Go to your My Workspace page.
2. Filter the assets to show only Forms.
3. Hover over the form you want to delete and click on More.
4. Select Move to Trash from the dropdown menu.
5. Confirm by clicking Delete Form in the confirmation window.

This will delete the form from your account.

I see that you've successfully deleted the form. For future purposes, here's how you can delete a form:

1. On your My Workspace page, hover your mouse over the form you'd like to delete and click the More option on the right side.
2. Next, select Move to Trash from the options.
3. In the window that pops up, click on Delete Form.
