I have a premade form that I would like to upload.
For now, just the Company Name is a fillable field for text (for testing)
There are a couple checkboxes beneath that for Organization Type.
I can't upload the PDF to Jotform. Can you assist?
I have attached the form.
Thank you. -

Hi Filippo_Erica,
Thanks for reaching out to Jotform Support. I'm an AI assistant. I'll do my best to help you until someone from the support team is available.
Regarding your issue with uploading a PDF with fillable fields, please ensure that your PDF meets Jotform's requirements for PDF uploads. Sometimes, certain fillable fields or checkboxes may not be fully supported if they are not created in a compatible way.
Let me show you how to upload your fillable PDF form correctly:
- Go to My Forms in your Jotform account.
- Click on Create Form and select Import Form.
- Choose Import PDF Form and upload your fillable PDF.
If the upload still fails, it might be due to the specific structure of your PDF. You can try recreating the form using Jotform's form builder to ensure full compatibility.
Give it a try and let us know how it goes.
Best regards,
Jotform Support -

The instructions don't say whether I need the fillable fields already created as fillable in a pdf software.
However, I have tried with and without, and doesn't upload. -

Hi Filippo_Erica,
I understand that you’ve tried uploading your PDF both with and without fillable fields, but it’s still not going through.
To help us look into this further, can you share the PDF file with us along with a screen recording of what’s happening so we can try to replicate it on our end? You can upload the file and video to any public sharing platform like Google Drive or Dropbox, or record the video using Loom and share the link with us.
Once we hear back from you, we'll be able to move forward with a solution.

The link to the file is below. No video needed, absolutely nothing happens when I try to upload.

Hi Filippo_Erica,
I imported your PDF form and tested it on my side, and everything worked as expected. Check out the screenshot below to see my results:
Can you try it again and see how it goes? If you run into the same issue again, here are a few quick troubleshooting steps you can try if you haven’t already:
- Clear your browser's cache.
- Try using a different browser or incognito/private mode.
Meanwhile, here are the steps for converting your PDF to a fillable form:
- On your My Workspace page, click on Create on the top-left side of the page.
- Select Form from the assets, and then click on Smart PDF Form.
- In Smart PDF Forms, click on Upload Document, and then select the PDF you want to convert to a form, or just drag and drop the PDF into the white upload area.
If some fields aren’t added automatically, you can manually add them to your form, and then match it to the PDF on the right side of the page.
That's it. Once you set everything up, when the form is filled, it will automatically fill your PDF file. Check out the screenshot below to see how it'll work:
Give it a try and let us know if you need any help. -
Filippo_EricaReplied on August 10, 2025 at 4:55 PMThank you. Didn't work using Chrome on 3 different computers in 2
different USA States.
It did work on Mozilla.
Now, it doesn't allow me to use my actual form, it seems. It converts it to
a jotform style entry?
Sincerely,
Erica Filippo

Hi Erica,
I understand that you are seeing your form converted into a Jotform-style entry rather than retaining the original PDF format.
This behavior is expected when using Jotform's Smart PDF Forms feature. When you upload a PDF, it converts the file into an interactive web form that matches your PDF's fields. This allows users to fill out the form online, and the data will automatically populate your original PDF file. So, the form you see in Jotform is the fillable web version of your PDF, not the PDF itself.
Since the upload works on Mozilla Firefox, I recommend using that browser for uploading and editing your PDF forms to avoid the issues you encountered on Chrome. Also, make sure to clear your browser cache or try incognito mode if you want to troubleshoot Chrome further.
If you want to keep the original PDF layout and use it as a fillable form, Jotform's Smart PDF Forms is the right approach, but it will present the form in the Jotform interface for filling. The filled data will then be merged back into your PDF.
Let us know if you need any more help.
