Card Form: How to edit the Welcome Page?

The CESLA sample screen is still in my form, NOT the one I am trying to change it to.

Thanks for reaching out to Jotform Support. You will need to update your Welcome Page, let me show you how:

1. In your Form Builder Page, go to Edit Welcome Page.

2. You can now customize the welcome text according to your requirement.
