Radio Buttons with Variable Input

    Maybe you can help me with this? Trying to get the following as an Element on the form.

    ---------------

    Please indicate below how you want your Pledge recognized:

    0 Anonymous

    0 Myself

    0 In Honor of _______________________

    0 In Memory of ______________________

    0 Other _____________________________

    ----------------

    The user will select only one of the options (Radio Button). Upon selecting any of the last 3 options, the user needs to put in an answer.

    Hi grpace777,

    Thanks for reaching out to Jotform Support. Unfortunately, it is not possible to add another field within the Single Choice element wherein submitters can input there answer aside from the Other option. As a workaround, you can add separate Short Text field that will show if they select any of the last three options, they will be prompted to enter additional information. Let me show you how to do it:

    1. In Form Builder, click on Add Element on the left side of the page.
    2. Under Basic tab, drag and drop Short Text element after the Single Choice element on your form.
    3. Click the gear icon on your Short Text field.
    4. Under General tab, toggle on the Required option.
    5. Go to Advanced tab and toggle on the Hide option. Radio Buttons with Variable Input Image 1 Screenshot 40
    6. Next, to setup the Condition, click on Settings on the orange navigation bar on top of the page.
    7. Click Conditions on the left side panel.
    8. Click Add Condition and select Show/Hide Field.

    Your answer of putting a short text with conditional formatting of the radio button worked. Thank you. Suggest putting it on the site as an answer to frequent asked question.
